    What is a Patient Pathway?

    A Patient Pathway is a clinically aligned pathway personalised to your condition and circumstances. It enables you to receive personalised communications from your healthcare professional to keep you informed as you progress through the multiple stages of your health journey.

    Patient Pathways make it easy to keep information and communications about your health journey in one convenient place. Receive documents, videos, tasks, messages and reminders. Complete questionnaires and forms to keep your care team informed.

    What is a Pathway Stage?

    The patient journey is divided into various stages of care along a pathway. For example, a typical surgery pathway includes stages such as triage, diagnosis, pre-op, surgery and recovery. These stages are used to deliver communications to you that is relevant to where you are in your journey.

    How do I go to the next Stage in a Pathway?

    Your Pathway may transition from stage to stage automatically, or you can choose to transition to the next stage in a pathway through the app using the following steps:

    1. Navigate to “My Profile” by tapping the Settings icon in the top-right corner of the Home screen.

    2. Tap “Proceed to next stage in a pathway”.

    3. A list of pathways will be displayed. Tap the Pathway you would like to transition to the next stage.

    4. A list of stages will be displayed. Your current stage will be indicated by the blue circular checkmark. Under the current stage heading tap “Proceed to next stage”. Please note, if you are in the final stage of a pathway you will not see the Proceed to next stage button.

    5. A message will now appear asking you to confirm that would like to update your journey by proceeding to the next stage. Tap “Update my customer journey” to confirm, or “X” in the top-right corner to cancel.

    6. Once your stage has been successfully changed, you will see a Success message at the top of the screen.

    How do I update my Key Dates?

    Your health journey will have key dates such as those for admission, and any procedures. You may have different “Key Dates” for different pathways. To View your Key Dates, go to the Calendar page.

    To assign or change the day of your Key Dates:

    1. Navigate to “My Profile” by tapping the Settings icon in the top-right corner of the Home screen.

    2. Tap “Set My Key Dates”.

    3. A list of key dates will be displayed. Tap the field under the heading of the key date you would like to update.

    4. Choose the date by selecting it from the data picker and tap “Ok”. Tap “Reset” if you don’t want to change the date at this time, then tap “Ok”.

    5. Tap “Set My Key Dates” to submit the new date.

    6. Once the date has been successfully changed, you will see a Success message at the top of the screen.

    When will I know my Pathway has had a Stage transition?

    Your Pathway may transition from stage to stage automatically. You may notice this in multiple areas of the app:

    • The home page will have a notification on the Timeline informing you of the Stage Transition.

    • The Library and Forms areas of the app will display new information, based on the new stage of your journey.

    How do I know what Forms need to be submitted?

    You may be required to fill out forms so that your care team can keep up to date with your current health status. This is done through the Forms section of the app.

    To see what forms require submission in your current stage, follow these steps:

    1. Navigate to “Forms” on the bottom navigation.

    2. The “Current Stages” tab on the Forms page shows all forms, questionnaires and tasks for your current stages.

    3. Once a form is completed, its status will change to have a “success tick”. If a form is listed in Current Stages without a success tick, it requires submission as soon as is possible.
