    Type 2 Diagnosis

    Provides key information for children and adolescents recently diagnosed with Type 2 diabetes and includes Baseline survey forms for Patient and Clinician and information about the National Diabetes Services Scheme (NDSS).

  • 2
    Type 2 Education

    This stage includes information about Type 2 diabetes including types of insulin, physical activity and blood sugar as well as the value of being part of a support group.

  • 3
    Type 2 Management

    Provides details about managing Type 2, how to take insulin, diabetes and mental health and information on eating well, maintaining a health weight and getting active.

  • Additional Content

    This pathway also includes general information about diabetes tests, the Diabetes Educators Association, GP Management Plans and Team Care Arrangements. A WHO Wellbeing Index PROM is included.

This pathway provides a care plan for children and adolescents with Type 2 diabetes. It includes an initial assessment survey to establish the patient’s baseline characteristics and review surveys to assess ongoing management. 

PATHWAY

The pathway has three stages

  1. Type 2 Diagnosis
  2. Type 2 Education
  3. Type 2 Management

as well as additional content that is not aligned to a stage but is available at all times.
