    Provides key information for persons recently diagnosed with Gestational Diabetes and survey forms for establishing a baseline and review management for Patient and Clinician.

    Gestational Diabetes Education

    This stage includes information about Gestational Diabetes including monitoring blood sugar, during labor, after birth fact sheets.

  • 3
    Gestational Diabetes Management

    Provides details about managing Gestational Diabetes, what to expect after the baby is born, diabetes and mental health and information on eating well, maintaining a health weight and keeping active.

  • Additional Content

    This pathway also includes general information about diabetes tests, the Diabetes Educators Association, GP Management Plans and Team Care Arrangements. A WHO Wellbeing Index PROM is included.

This pathway provides a care plan for women with Gestational diabetes. It includes general information about Gestational diabetes including tests and risk factors, an initial assessment survey to establish the patient’s baseline characteristics and review surveys to assess ongoing management. 

PATHWAY

The pathway has three stages

  1. Gestational Diabetes Diagnosis
  2. Gestational Diabetes Education
  3. Gestational Diabetes Management

as well as additional content that is not aligned to a stage but is available at all times.
